Social Prescribers
We know through our own lived experience and by listening to patients that many things affect your health and wellbeing.
Our Social Prescribing Team
Our GPs tell us that a lot of people visit them feeling isolated or lonely or they are stressed out by work, financial and housing problems, or the stress of managing different long-term health conditions.
Other people are seeking support to help them live a healthier happier life, they know they need to eat better, exercise more, make more social connections but aren’t sure where to start.
That’s where social prescribing comes in. It starts with a conversation. It might be the conversation you’ve just had with your Doctor or Nurse. They can refer you to the social prescribing team who are here to listen to you and put you in touch with the people and activities that might help you to feel better.
Your Social Prescriber might introduce you to a community group, a new activity or a local club. Or they might help sign post you to find legal advice or debt counselling. They might just help you find information and guidance on your situation or what local resources there are.
Social prescribing can help you to have more control over your own health and find ways to improve how you feel in a way that suits you. Studies show that people get better and feel better faster than those treated with medicine alone. And because it works, it’s happening more and more in the NHS – including here in our Surgery.
If you would be interested in being referred for Social Prescribing then please talk to your Doctor, Nurse, HCA or come and have a chat with us over a cup of tea or coffee at our Wednesday morning Helping Hands sessions 10:30am to 12:30pm we have an information table on activities happening in the local area and the opportunity to have a chat over a cup of tea or coffee.
If that’s not convenient, contact the practices Social Prescribers Jo Lynch or Jackie Tumelty.
